:: The Admission Process
Admission Requirements
Admission at SMC will be on a competitive basis
with each applicant going through a selective entrance test. The
school desires to admit students who will give evidence of average
and/or more than average ability and achievement as well as capacity
for continuing their education.
FOR LABORATORY SCHOOL:
Admission Requirements
1. Recommendation/Certificate of Enrollment
2. Birth Certificate
(photocopy, please bring original)
3. Report Card/Certification of Grades
(photocopy)
4. Income Tax Return/BIR Certification
(photocopy)
5. 3-pcs 1"x1" ID picture
FOR GRADE SCHOOL and HIGH SCHOOL:
Application Requirements
1. Photocopy of the report card with the 2nd Quarter marks indicated
2. Birth Certificate (show original and submit a photocopy only)
3. Grade/Year placement for those who studied abroad to be secured from the DepEd.
4. Certificate of Good Moral Character (for High School Students)
5. Two identical 1"x1" ID pictures
Application Procedure
1. Present application requirement at the Registrars Office
2. Secure Application Form and Entrance Examination Slip from the Registrars Office
3. Pay the testing fee at the Cashier. This fee is not a guarantee of admission and is not refundable.
4. Take the entrance test on the scheduled date.
5. Wait for the results which will be given out a week after the test.
6. For successful examinees, be ready to be interviewed on schedule.
Enrollment Requirements
1. Original Report Card properly signed by the principal of the school where the student came from.
2. Accomplished personal data sheet for student.
3. Confirmation Slip signed by both parents/guardian and student that they are willing to meet the
school requirements and to observe existing school rules and regulations and those which will be
implemented in the future.
